A S\u00edndrome da Insufici\u00eancia Adquirida (Aids), causada pelo v\u00edrus HIV, pode afetar diversas regi\u00f5es dos olhos, desde a p\u00e1lpebra at\u00e9 a retina, e provocar manifesta\u00e7\u00f5es locais.<\/p>\n

Isso acontece porque o v\u00edrus reduz as c\u00e9lulas de defesa do organismo, desajustando a imunidade que antes regulava processos, como a divis\u00e3o celular e prolifera\u00e7\u00e3o de agentes infecciosos, principalmente em est\u00e1gios mais avan\u00e7ados.<\/p>\n

Dentre as manifesta\u00e7\u00f5es, a mais frequente \u00e9 a retinopatia por HIV. Sendo progressiva, \u00e9 fundamental o acompanhamento do paciente por um oftalmologista para preven\u00e7\u00e3o e detec\u00e7\u00e3o precoce de poss\u00edveis complica\u00e7\u00f5es nos olhos.<\/p>\n

Retinopatia por HIV \u2013 o que \u00e9<\/h2>\n

Tamb\u00e9m conhecida como retinopatia da Aids, a retinopatia pelo HIV causa exsudatos algodonosos (edema localizado, secund\u00e1rio a infarto da camada de fibras nervosas, que se apresentam como borr\u00f5es brancos parecidos com flocos de algod\u00e3o). Pode provocar tamb\u00e9m pequenas hemorragias na retina, microaneurismas e teleangectasias.<\/p>\n

Isso se deve ao dano \u00e0 isquemia retiniana, secund\u00e1ria a processos vasculares. Diversos estudos sobre os pequenos vasos da retina afetados pelo v\u00edrus demonstram edema das c\u00e9lulas endoteliais, duplica\u00e7\u00e3o da l\u00e2mina basal e perda de pericitos. Desse modo, aponta-se para a quebra da barreira uveovascular como mecanismo patol\u00f3gico da manifesta\u00e7\u00e3o.<\/p>\n

A exata patogenia ainda \u00e9 desconhecida. A hip\u00f3tese \u00e9 que seja multifatorial: combina\u00e7\u00e3o de dep\u00f3sito de imunocomplexos, altera\u00e7\u00e3o vascular decorrente da infec\u00e7\u00e3o pelo v\u00edrus, les\u00e3o local por elementos citot\u00f3xicos e anormalidades reacionais.<\/p>\n

Pesquisas recentes apontam para a maior preval\u00eancia de les\u00f5es do segmento posterior e baixa acuidade visual em pacientes com n\u00edveis menores de linf\u00f3citos T com CD4. Na verdade, a contagem de CD4 menor que 200 c\u00e9lulas mm\u00b3 e idade maior do que 35 anos foram os principais fatores de risco independentes para a retinopatia pelo HIV.<\/p>\n

Geralmente, os exsudatos desaparecem naturalmente entre duas e seis semanas e n\u00e3o deixam sequelas na vis\u00e3o. Na maioria dos casos, n\u00e3o apresentam sintomas iniciais.<\/p>\n

O grande perigo \u00e9 a abertura para o surgimento de infec\u00e7\u00f5es oportunistas, como a retinite por citomegalov\u00edrus (CMV). Estima-se que 45% dos pacientes com retinopatia pelo HIV desenvolvam o quadro. A complica\u00e7\u00e3o ocorre geralmente quando a contagem de CD4 est\u00e1 abaixo de 100\/mm\u00b3.<\/p>\n

Retinopatia pelo HIV \u2013 diagn\u00f3stico<\/h2>\n

Como \u00e9 assintom\u00e1tico na maioria dos casos, o paciente com HIV precisa ser acompanhado por um oftalmologista. O diagn\u00f3stico de retinopatia pelo HIV \u00e9 realizado por exame de fundo de olho. Neste caso, os achados da doen\u00e7a s\u00e3o parecidos com os de outras retinopatias, como as derivadas do diabetes e de hipertens\u00e3o arterial, e necessita de um olhar mais atento do especialista.<\/p>\n

Al\u00e9m disso, o diagn\u00f3stico pode ser feito por um m\u00e9dico localizado em qualquer lugar do mundo.<\/p>\n

Mais de 40 mil exames j\u00e1 foram realizados com o Eyer em todo o Brasil. Ao todo, h\u00e1 43 mil pacientes cadastrados do SUS, a\u00e7\u00f5es sociais e mutir\u00f5es, como o Mutir\u00e3o de Diabetes de Itabuna (BA), consult\u00f3rios e institui\u00e7\u00f5es como USP, Unifesp, Hospital Albert Einstein, Santa Casa de S\u00e3o Paulo e Bayer, dentre outros.<\/p>\n

Mesmo com tecnologias de ponta aplicadas na produ\u00e7\u00e3o do aparelho, a portabilidade e o tamanho reduzido permitem que o Eyer apresente um custo at\u00e9 dez vezes mais baixo em rela\u00e7\u00e3o aos retin\u00f3grafos tradicionais.<\/p>\n

Tratamento<\/h2>\n

O tratamento da doen\u00e7a n\u00e3o \u00e9 espec\u00edfico, sendo que a introdu\u00e7\u00e3o da terapia anti-retroviral (TARV) tem minimizado muito o problema. Pesquisas demonstram diminui\u00e7\u00e3o de at\u00e9 95% na incid\u00eancia de algumas complica\u00e7\u00f5es oculares relacionadas ao HIV com o uso da TARV.<\/p>\n

Apesar desta redu\u00e7\u00e3o, a averigua\u00e7\u00e3o de les\u00f5es oculares em pacientes com HIV permanece muito importante. Isso porque pode trazer preju\u00edzos \u00e0 fun\u00e7\u00e3o visual, como a cegueira, quando surgem infec\u00e7\u00f5es oportunistas, como a retinite por CMV.<\/p>\n
